Registered Nurse A New Hope Home Health Care

Job Summary
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. The RN will provide high-quality patient care in the comfort of patients' homes, focusing on promoting health, preventing illness, and managing chronic conditions. This role requires strong clinical skills, excellent communication abilities, and a commitment to delivering personalized care to diverse patient populations, particularly in geriatrics.

Responsibilities

• Conduct comprehensive assessments of patients’ health status and develop individualized care plans.
• Administer medications and treatments as prescribed, ensuring adherence to safety protocols.
• Provide ostomy care, pain management, and health coaching to enhance patient well-being.
• Monitor patients' progress and adjust care plans as necessary in collaboration with interdisciplinary teams.
• Educate patients and their families on disease management, medication administration, and use of Durable Medical Equipment (DME).
• Perform case management duties including coordinating services and resources for patients.
• Document all patient interactions accurately in compliance with healthcare regulations.
• Maintain a safe environment for patients by adhering to infection control protocols.

Requirements

• Current Registered Nurse (RN) license in the state of practice.
• Experience in home health care or hospital settings preferred.
• Strong skills in geriatrics and case management are highly desirable.
• Proficiency in medication administration and pain management techniques.
• Ability to lift patients safely and assist with mobility as needed.
• Basic Life Support (BLS) certification is required.
• Excellent communication skills and a compassionate approach to patient care.
• Must be able to work independently while managing time effectively.
