A New Horizon Health Care, LLC provides services to individuals with disabilities in Wake County, Vance County, Durham County surrounding areas. We are currently seeking caring and compassionate people to care for these great people. Full-time and Part-time or PRN positions available on all three shifts various positions available!! Most shifts are Monday - Friday 8a-4p,4p-12a &12a-8a open no weekends or mandated shifts. Weekly pay every Wenesday !!
Must have a GED or High School Diploma, or Transcripts valid drivers license be able to pass or obtain or already have a clean Child Clearance and FBI criminal background checks pass a drug screen 2 step TB Test (with in 3 months)CPR / First Aid(valid)Blood borne pathogens have medication certification (valid) .Proof of Covid 19vaccines and boosters are required along with Flu shots!!!!!!!!!

- Key Responsibilities :
- Assist individuals with daily living activities, including personal hygiene, meal preparation, medication management, and household tasks.
- Support individuals in achieving their personal goals and objectives as outlined in their Individual Support Plan (ISP).
- Facilitate social and recreational activities that promote community integration and enhance social skills.
- Foster a positive and supportive environment that encourages independence, dignity, and empowerment.
- Monitor and document individuals’ progress and behaviors, reporting any changes or concerns to the appropriate professionals.
- Encourage and assist individuals in developing life skills, decision-making abilities, and self-advocacy.
- Collaborate with families, caregivers, and multidisciplinary teams to ensure comprehensive support.
- Ensure compliance with all relevant regulations, policies, and best practices in care provision.
- Provide transportation to appointments, community activities, and social events as needed.
- Participate in training and professional development opportunities to enhance skills and knowledge in the field.
