New Leaf's Mesa Community Action Network – Client Services program is seeking a dedicated and compassionate Bilingual Eligibility Specialist/Case Manager to join our team. This role plays a critical role in assisting individuals and families in crisis by determining their eligibility for various support programs. This includes conducting assessments, reviewing documentation, and providing guidance on available resources such as rental assistance, utility support, and food services. The specialist works closely with clients to create action plans, connect them with community resources, and ensure timely access to essential services. As part of a collaborative team, this role will contribute to empowering clients to achieve greater stability and self-sufficiency.
Hourly Pay Rate: $18.50
Schedule: Monday - Friday; 8am - 5pm
Location: Mesa, AZ (in-office)
QUALIFICATIONS
Education:
High School or GED required
Associates in a related field preferred
Experience:
Two (2) years’ experience in a related field required
Knowledge of community resources
Bilingual verbally in Spanish, highly preferred.
DUTIES AND RESPONSIBLITIES. Responsibilities will include but not limited to:
Handle all tasks related to active case management, including benefit eligibility and disbursement, creating and maintaining client files, following up on client status, and recommending other MesaCAN program opportunities.
Review phone messages, contact clients, conduct phone assessments, verify clients' income eligibility, and inform them of all required documents.
Conduct intake screenings and eligibility determinations for crisis prevention program services, and perform basic needs assessments for potential clients as relevant to program and client needs.
Schedule, facilitate, and/or participate in Family/Individual and Case Management meetings as appropriate, and update and complete staffing notes.
Initiate active case management for clients requesting additional MesaCAN services, and coordinate services with clients/families and other service providers.
Coordinate consumer evaluations and re-assessments in accordance with agency policies and all accreditation and regulatory agency requirements.
Collaborate to arrange appropriate referrals and community-based services.
Assist consumers and families in accessing community services and entitlements, provide referrals to external services and agencies as needed, including identifying funding sources and connecting them to other agencies/services.
Consistently seek out and develop strategies to identify new clients in need of the services offered at MesaCAN.
Access community resources to obtain assistance as needed. Complete case documentation and agency paperwork accurately, timely, and thoroughly.
Coordinate services with clients, families, and other service providers, and provide information to help clients and families research and achieve their goals.
Provide information as needed to help clients and families research and pursue their goals.
Attend and participate in program staff meetings and any other meetings as directed by your supervisor(s).
Communicate with community partners as needed for program and client requirements.
Schedule consumer appointments and obtain required authorization (as relevant to program/client needs).
