Position Summary
We are seeking a highly motivated and detail-oriented Clinical Coordinator (RN) to join our team in a remote capacity. The Clinical Coordinator will play a key role in overseeing the quality and compliance of clinical operations, including orders management, quality assurance (QA), auditing, and adherence to regulatory requirements. The ideal candidate will have strong leadership skills, experience with Axxess EMR , and a thorough understanding of ACHC accreditation standards and Indiana state home health regulations .
Key Responsibilities
Clinical Operations & Orders Management :
- Manage and oversee all physician orders, plan of care (POC) updates, and coordination of services to ensure timely completion.
- Verify accuracy and completeness of physician-signed orders, certifications, and recertifications in accordance with regulatory guidelines.
- Monitor timely submission and follow-up on outstanding physician orders to prevent delays in patient care.
- Conduct chart audits to ensure compliance with ACHC, CMS, and Indiana state regulations.
- Monitor clinical documentation for accuracy, completeness, and adherence to agency policies .
- Identify documentation deficiencies and provide feedback / training to field clinicians as needed.
- Implement quality improvement initiatives to enhance patient care and compliance.
- Ensure adherence to ACHC accreditation standards and Indiana home health regulations.
- Assist in preparing for state and accreditation audits , ensuring compliance with all necessary policies and procedures.
- Stay up to date with regulatory changes and educate staff on updated protocols and best practices.
- Utilize Axxess EMR to review, manage, and audit patient records for accuracy and compliance.
- Provide support and training to clinical staff on proper documentation and system utilization .
- Ensure that documentation meets Medicare, Medicaid, and private insurance requirements .
- Work closely with case managers, field clinicians, and administrative staff to ensure seamless coordination of patient care.
- Serve as a resource for clinical staff, answering questions related to documentation, compliance, and regulatory standards.
- Participate in weekly case conferences and interdisciplinary team meetings to discuss patient care plans and compliance updates.
- Support onboarding and training of new clinicians to ensure understanding of agency protocols and documentation expectations.
- Active, unencumbered RN license in Indiana (required).
- Minimum 2 years of home health experience (preferred).
- Experience with Axxess EMR (preferred) or similar home health software.
- Strong knowledge of ACHC accreditation standards, Indiana home health regulations, and CMS guidelines .
- Experience in orders management, chart audits, QA, and compliance oversight .
- Excellent organizational, analytical, and problem-solving skills.
- Ability to work independently in a remote environment while maintaining productivity and accountability.
- Strong communication and leadership skills to collaborate effectively with interdisciplinary teams.
