A DAY IN THE LIFE OF A REGISTERED NURSE CASE MANAGER
As a Registered Nurse, you play an essential role in conducting patient visits to assess their nursing needs. You are in charge of making the initial evaluation visit in order to develop the plan of care for each of your patients. As you continue to visit your patients, you perform ongoing comprehensive assessments of their needs while making any necessary revisions to the plan of care under the direction of the physician. Using your specialized nursing skills, you provide whatever services or treatments are needed as well as educate the patient or caregiver regarding the disease process, self-care techniques, and prevention strategies.
In addition, you initiate the appropriate preventive and rehabilitative nursing procedures as ordered by the physician. You also prepare clinical and progress notes as well as coordinate all relevant services while making referrals when needed. Through direct communication with physicians and other staff members, you ensure that all necessary parties are kept informed about the condition and needs of each patient in your care. You continually re-evaluate the nursing needs of patients and assist with the discharge procedure when they are ready. At times, you provide supervision to our LPNs, vocational nurses, and home health aides, which involves delegating tasks and teaching them new skills.
The safety of your patients is of the utmost importance to you, and you take great care to practice infection control by following universal precautions. You also actively recognize and report any forms of abuse according to policy as well as federal and state laws. As needed, you help with tasks around the office and assist with leading orientation for new nurses. QUALIFICATIONS FOR A REGISTERED NURSE CASE MANAGER (RN)
- Graduate of an approved school of professional nursing
- Active RN license in the state of Oklahoma
- 1 years of nursing experience
- CPR certification or the ability to obtain one within 60 days of being hired
- Valid driver's license, reliable transportation, and proof of insurance
- Ability to travel long distances and adapt to frequently changing schedules
Experience with home health care, critical care, or in an emergency room is preferred but multiple factors will be taken into consideration.
