Duties

- Facilitate communication between patients, families, and medical staff to ensure coordinated care.
- Educate patients and families about available resources, including community services and support groups.
- Utilize motivational interviewing techniques to empower patients in their care journey.
- Maintain accurate documentation of patient interactions and progress in accordance with medical terminology standards.
- Stay informed about ICD-10 codes relevant to patient diagnoses for accurate reporting and billing.
