Job Description Summary:
The in-home caregiver is a member of the home care team who works under the supervision of the RN Supervisor to perform various personal care services as necessary to meet the client’s needs, according to the plan of care. The in-home caregiver is responsible for observing clients, reporting these observations, and documenting observations and care performed.
The caregiver will be assigned in a manner that promotes quality, continuity, and safety
of a client’s care.
Essential Job Responsibilities:
Responsibilities of the caregiver include, but are not limited to the following:
- Personal care tasks to include:
- Bathing;
- Grooming, including oral care;
- Assist the client with dressing and undressing;
- Assist the client with toileting activities;
- Assist in feeding the client, if necessary;
- Feed clients who are unable to feed themselves;
- Take and record vital signs (temperature, pulse, respirations and blood pressure);
- Measure and record height and weight, if necessary;
- Support good skin care to prevent skin breakdown (i.e. turn and reposition client, actively encourage movement);
- Assist client with active and passive range of motions as defined by the plan of care;
- Provide client with help walking and transferring;
- Observe client’s physical, mental and emotional conditions;
- Report any change to the supervisor;
- Document observed changes;
- Perform light housekeeping;
- Perform meal preparation and clean up;
- Change bed linens and make up the client’s bed;
- Maintain an orderly client environment.
- Follow principles of infection control and Universal Precautions;
- Identifies and participates in performance improvement activities;
- Provide transportation to doctor appointments, outside activities, shopping, etc.; if included in care plan
- Participates in client teaching according to the client’s plan of care;
- Assist in providing a safe environment and promote quality patient care through adherence to established policies, procedures and standards;
- Ensure client’s rights are adhered to. These include but are not limited to the right to:
- Privacy and dignity while care is being provided;
- Accept or refuse care;
- Confidentiality of their information;
- Respond rapidly and appropriately in an emergency situation;
- Use equipment properly and in a safe manner.
Qualifications:
- High school diploma, GED required;
- Driver's License required with 3 years of driving experience
- Successful completion of a PCA/ HHA / CNA from an approved provider.
- Compliance with Virginia Board of Nursing state registry requirements, with current license:
- Minimum one (1) year of documented experience required;
- Must consent to and pass a criminal background check;
- Have a baseline and annual TB test on record with A Place At Home Arlington & Alexandria;
- Have a Free from Communicable Disease form on record with A Place At Home Arlington & Alexandria;
- Self-directing with the ability to work with little direct supervision;
- Demonstrate effective oral and written communication skills;
- Ability to express spoken and/or written ideas in English;
- Works as a team member.
Job Limitations:
The caregiver will not function in any manner viewed as the practice of nursing according to practice acts. Specifically, the caregiver will not administer medications, take physician’s orders, or perform procedures requiring the training, knowledge and skill of a Licensed Nurse.
Working Conditions:
Contact with clients in their living environment. May be required to respond in an emergency situation. Travel required. Position includes tasks that have the potential for a medium level of exposure to blood and other potentially infectious material (OPIM) such as body fluids/tissues..
Personal Protective Equipment:
May include gloves, mask, eye protection, CPR shield and disposable outer covering.
Physical Requirements:
Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull. Must be able to lift up to 50 pounds.
