What are the responsibilities and job description for the Case Coordinator position at A Plus Care?
A-Plus Care is currently seeking a motivated and experienced case coordinator to join our successful, team-oriented staff as a full time employee.
The Coordinators are expected to use critical thinking and organizational skills when helping both patients , HHAS's and PA's for homecare services.
DUTIES:
- Educate and guide potential/active PA's to meet/maintain agency and state compliance, including consistent follow-ups for HR and DOH requirements.
- Log both patient and employee activity in HHAeXchange
- Communicate all changes in patient services and conditions to insurance plan
- Update and maintain patient calendar regularly; scheduling home attendants to patient calendar, confirming attendance and POC duties
- Assisting Payroll Department with collecting timesheets and other pre-billing issues for patients
REQUIREMENTS:
- Fluent in Spanish
- coordination experience required
- Proficient in Customer service and customer satisfaction skills.
- Organized and punctual
