Mission:
The core mission of the Clinical Manager is to oversee the day-to-day support of the field team and the patient experience. They are a key member of the agency clinical leadership team and ensure that clinical plans and best practices are implemented and integrated on the front lines to enable the field team to provide quality patient care and patient / caregiver experiences. They are responsible for creating a high-performing field team and are critical to ensuring their agency is the best place to work and receive care. Provides supervision of Clinical Admin to ensure patient and field team members needs are met.
Job Accountabilities include but are not limited to:
- 100% of field team understands and follows the iOS process
- Review schedule requests and approve or deny based on patient needs vs care insights recommendations
- Complete ride-alongs and provide feedback at least 1x/year
- High patient satisfaction by being above average on two key HHCAHPS and google 5 star reviews
- Ensures each team member is educated and compliant with the policy of charting visits within 24 hours
- Carries out Semi-Annual Assessments for field team
- Consistent and reliable presence in messaging app proven by prompt responses to field team
- Prompt resolution to patient/family inquiries, complaints, and requests
- Coordinates orientation and training of field team members
- Lead case conference and carry out specific education topics, under the direction of the DPCS
- Clinical operations align with policy and procedures / best practices
- Staffing plan implemented, under the direction of the DPCS, to meet patient needs
- Seamless integration with community and contracted partners
- Coordinate field team needs with Clinical Director, ED, and Scheduler
- Support QAPI efforts
- Manage and collaborate with clinical admin
