Job Overview
We are seeking a dedicated and compassionate Case Manager Hospice Registered Nurse to join our healthcare team! This role involves working closely with patients, families, and healthcare providers to develop and implement individualized care plans. The Registered Nurse Case Manager Hospice leader will play a critical role in navigating complex medical situations.
Duties
- Conduct comprehensive assessments of patients' medical histories and current health status.
- Develop, implement, and evaluate individualized care plans based on diagnostic evaluations.
- Collaborate with multidisciplinary teams to ensure continuity of care across various settings, including PICU, medical-surgical units, and clinics.
- Monitor patients' vital signs and overall progress, adjusting care plans as necessary.
- Advocate for patients’ needs while ensuring compliance with HIPAA regulations.
- Provide education and support to patients and families regarding treatment options and resources available.
- Facilitate communication between healthcare providers, patients, and families to promote effective care coordination.
- Supervise and support LPNs and Aides.
Experience
- Current Clean Registered Nurse (RN) licensure is required in OR.
- Excellent communication skills with the ability to work collaboratively in a team environment.
- Ability to manage multiple priorities effectively while maintaining attention to detail.
- 1 Plus years of nursing hospice experience.
