Job Summary
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. Duties
- Conduct comprehensive assessments of patients' health status, including physical, emotional, and social needs.
- Develop and implement individualized care plans based on patient assessments and established protocols.
- Administer medications and treatments as prescribed by physicians while ensuring adherence to safety standards.
- Monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes.
- Provide education and support to patients and their families regarding health management, medication administration, and lifestyle modifications.
- Maintain accurate documentation of patient care activities in compliance with HIPAA regulations.
- Stay informed about best practices in home health nursing, including trauma medicine, geriatrics, behavioral health, and diagnostic evaluations.
Requirements
- Valid Registered Nurse (RN) license in the state of practice.
- Strong understanding of HIPAA regulations and commitment to maintaining patient confidentiality.
- Excellent interpersonal skills with the ability to communicate effectively with patients, families, and healthcare professionals.
- Ability to work independently while managing multiple priorities in a home health setting.
