Certified Home Health Aide (CHHA)

A Plus Home Health Care is a Medicare-certified agency, and we are seeking a dedicated and compassionate Certified Home Health Aide to join our healthcare team, we're looking for a passionate candidate with a strong background in patient care and experience in various nursing specialties. This role is critical in providing high-quality one-on-one skilled nursing care to patients across different settings, for example, patients' homes and various facilities. The Certified Home Health Aide will assess patient needs, play a critical role in providing comprehensive care and support to individuals who require assistance with their daily activities and personal care need, collaborate with family members or other healthcare professionals to ensure optimal patient outcomes.

Job Overview
As a Certified Home Health Aide, you will play a vital role in providing compassionate care and support to individuals in their homes or assisted living facilities. Your primary responsibility will be to assist clients with daily living activities, ensuring their comfort and well-being. You will work closely with clients who may have disabilities, memory care needs, or require assistance with vital signs and meal preparation. This position is ideal for individuals who are dedicated to enhancing the quality of life for others.

Duties

• Provide personal care assistance, including bathing, grooming, and dressing.
• Assist clients with mobility and transfers to ensure safety and comfort.
• Monitor and record vital signs as needed.
• Prepare nutritious meals according to dietary restrictions or preferences.
• Offer companionship and engage clients in meaningful activities.
• Assist with medication reminders and ensure adherence to prescribed regimens.
• Support clients in maintaining a clean and safe living environment.
• Communicate effectively with healthcare professionals regarding any changes in client conditions.

Qualifications

• Experience in assisted living, home community care, or home health settings is preferred.
• Knowledge of memory care techniques and working with individuals with disabilities is a plus.
• Familiarity with basic health systems and the ability to monitor vital signs.
• Certification in CPR is highly desirable.
• Strong interpersonal skills and a compassionate approach to caregiving.
• Ability to follow care plans and adapt to the individual needs of clients.
• Reliable transportation for travel between client locations may be required.
