Our Medical Social Worker reports to the Chief Clinical Officer (CNO) and performs the following duties to support Edgerton Hospital:
- Develops, implements, and directs the policies and procedures of the Patient and Family Service Department.
- Collaborates with social workers at other hospitals in regards to admissions and discharges.
- In partnership with the Chief Clinical Officer, determine budget, purchasing priorities, and department goals/objectives.
- Coordinates Swing Bed Admissions with Nursing/Therapies/Pharmacy and Registration.
- Provides patient/patient representative with Swing Bed Agreement and Patient Rights. The Social Work consult to be completed within 48 hours of admission.
- Attends multi-disciplinary meetings three times weekly with nursing, therapies, utilization review, pharmacy, and other departments as indicated, to address patients progress, concerns, and recommendations.
- Keeps patient/patient representative involved in discharge planning.
- Will assist patients to determine how they would like to make decisions about health care and who, if anyone they want involved in those decisions.
- Coordinates and facilitates care conferences.
- Consults with patient and/or patient representative to assess the psychosocial factors associated with patient’s illness/disability and to determine capacity/potential to cope with the planned care and treatment.
- Provides recommendations and options post discharge.
- Attends multi-disciplinary meetings three times weekly with nursing, therapies, utilization review, pharmacy, and other departments as indicated, to address patients progress, concerns, and recommendations.
- Attends patient rounding.
- Acts as advocate for Patient Rights.
- Provides crisis intervention in Emergency Department. (when on EHHS grounds)
- Works cooperatively with Police/Crisis/ADRC in any reported/suspected abuse or financial exploitation.
- Makes arrangements for medical equipment, adaptive equipment, clothing or personal items.
- Will assist staff in keeping patient and patient/patient representative informed about health statues, choices and ramifications of choices.
- Will assist staff to best meet the individual needs of each patient, including physical, emotional, behavioral, spiritual, and cultural needs.
- Responds to Medical Emergency.
- Maintains confidentiality and security of information.
- Provides education regarding Advance Directives to patients or others who would like assistance in completing Advance Directives (including staff and community members).
- Participates in the development and implementation of quality improvement and continuing assessment activities pertinent to the Patient and Family Services department.
- Demonstrates knowledge, skills, and understanding of the needs of adults, older adults, child and adolescents; including special considerations for individuals with disabilities and intellectual handicaps.
- Develops and maintains good rapport with hospital staff, and outside community referral agencies, to best meet the needs of the patients.
- Keeps abreast of current CMS/Joint Commission and state regulations, as well as professional standards within NASW.
- Consults with the multi-disciplinary team upon admission to begin identifying a discharge plan with will meet the patient’s needs.
- Participates in marketing efforts of EHHS.
- Participates in regularly scheduled health fairs and in-service training.
