Responsibilities include:
1. Is responsible for all developing person-centered implementation strategies in collaboration with the Interdisciplinary Team (IDT) who includes but iis not limited to the guardian, individual receiving services, DSP/caregivers, individual service coordinators, residential agency representatives, and other interested stakeholders.
A. Trains A Step Forward Community Day Services, ASF CDS Direct Support Personnel, DSP on the rational and the teaching methods behind each person-centered plan developed on their case load.
B. Identifies inconsistencies in training approaches or programs not being implemented as written and facilitates the resolution of these inconsistencies.
C. Monitors and assures follow-ups for all recommended services; supplies needed to run programs are provided in a timely manner to meet the needs of each individual.
2. Completes Annual Implementation Strategies and monthly progress reports on each strategy.
3. Effectively promotes competent interactions of direct care staff with each individual in program implementation and behavior management; and
4. Effectively coordinates with professional; paraprofessional and non-professional staff to provide direct and indirect professional services to meet the need of each individual.
Qualifications: At least one year of experience working with directly with individuals with developmental disabilities AND is one of the following:
A. A registered nurse; or
B. An individual who holds a bachelor’s degree in a professional category such as occupational therapy, physical therapy; psychology; social work speech-language pathology; recreational therapy; dietitian or be designated as a human services professional (sociology; special education; rehabilitation counselling).

- Strong understanding of resident rights and advocacy principles.
- Proficient in case management practices with the ability to develop effective care plans.
- Experience in conflict management and resolution techniques.
- Knowledge of dementia care practices, including memory care strategies.
- Ability to perform basic nursing tasks such as monitoring vital signs.
- Excellent communication skills for interacting with residents, families, and team members.
- Organizational skills to manage multiple cases effectively while maintaining attention to detail.
- Valid driver's license for transportation needs related to client services.
