Position Title: Day Habilitation Nurse RN
Reports to: VP of Clinical Services and Program Administrators
Position Summary:
Under the guidance of the VP of Clinical Services and program administrators, the Day Habilitation Nurse is a registered professional responsible for all nursing services provided to individuals within the program. This role involves diagnosing and treating health issues, delivering health education, and providing life-supportive and restorative care. The Nurse also carries out medical protocols prescribed by licensed physicians or dentists, ensuring individuals’ health needs are met consistently and compassionately.
Key Responsibilities:

Licensure: Maintain active licensure with the New York State Department of Education.

Medication Management: Prepare and administer medication to individuals, adhering to medical protocols.

Emergency Response: Manage and assess emergencies, ensuring appropriate care and quick response.

Health Assessment: Regularly assess and follow up on the health status of ill individuals as reported.

Medical Referrals: Ensure follow-up on referrals made by physicians or other service providers.

Physician Collaboration: Consult with attending physicians on specific medical concerns and issues.

Reviews and Documentation: Attend semiannual and annual reviews, preparing relevant medical information as needed.

Admission Assessments: Review medical profiles of new admissions and recommend any additional medical requirements.

Vital Statistics Maintenance: Track and record weights, vital signs, and other health data for individuals as necessary.

Hospitalization Follow-Up: Obtain medical records post-hospitalization and follow up on individuals’ status.

Dietary and Medical Updates: Keep staff informed about individuals' diets, restrictions, and any ongoing medical issues.

Medication Orders: Maintain a record of doctors’ orders for all individuals requiring medication at the program.

Medication Side Effects Education: Educate staff on potential side effects of psychotropic medication.

Annual Physical Exams: Ensure annual physical exams are conducted and documented for Level of Care eligibility.

Self-Medication Assessment: Evaluate individuals’ capacity to self-administer medication.

Medication Training: Train individuals in medication administration to foster independence.

Tuberculosis Testing: Oversee annual TB testing and ensure results are documented and filed.

AMAP Oversight: Supervise AMAP staff, verify certification status, and ensure timely recertification.

Staff Meetings Participation: Contribute medical insights and updates during staff meetings.

Infection Control: Manage communicable disease control by identifying risks, reporting to authorities, and implementing preventative measures.

Family Support: Assist families in locating appropriate medical services within the community upon request.

Direct Care Staff Training: Participate in planning and implementing training for direct care staff in essential areas:
- Illness Detection: Educate staff on recognizing signs that require medical intervention.
- Basic Health Skills: Teach personal hygiene, family life education, sex education, family planning, and venereal disease counseling.
- First Aid Training: Provide training in first aid response for accidents or illnesses.
- In-Service Documentation: Maintain written records of all training sessions, including dates and evaluations of effectiveness.

Qualifications
Qualifications:

Education & Licensure:
- Current NYS RN license; BSN preferred.
- CPR and First Aid certified.

Experience:
- 2 years in nursing, preferably with individuals with intellectual or developmental disabilities.

- Proficient in health assessment, medication management, and infection control.

Communication & Interpersonal Skills:
- Strong communication and empathy for vulnerable populations.

- Skilled in record-keeping and knowledgeable of NYS healthcare regulations.
