Nurse Practitioner
Job purpose
The Nurse Practitioner provides professional medical services for AACI Health Center patients. This position assesses, diagnoses, and treats patients, prescribes and dispenses medications, educates patients on illness, disease, and treatments; documents treatment and maintains clinical records, and participates in quality improvement processes. The Nurse Practitioner will provide coverage for other clinicians when needed and work as part of an integrated care team.
30-40 hour weekly schedules are available.

Duties and responsibilities
- Provide continuing, comprehensive healthcare maintenance and medical care to patients, including preventive, behavioral, and community health care.
- Provide accurate assessment, diagnosis, and treatment based on the ordered medical history, physical examination, and diagnostic tests.
- Prescribe and dispense medications.
- Develop and implement treatment plans with patients and other care team members.
- Educate patients on topics such as disease process, medication, prevention, and compliance.
- Refer patients to affiliated subspecialists when indicated.
- Document and maintain clinical records per appropriate standards and regulations.
- Participate in on-call after hours rotation.
- Provide coverage for other clinicians when needed.
- Participate in clinical quality improvement processes, including peer reviews.
- Maintain certification, professional training, and continued medical education.
- Participate in team meetings, data collection, health outcomes reporting, clinical audits, and program evaluation.
- Complete other related duties and projects as assigned.
Qualifications

Education & Experience:
- Master's Degree in Nursing or Physician Assistant.
- Minimum of two years of clinical work-related experience.
- Active, unrestricted license with the California Board of Registered Nursing as a Nurse Practitioner.
- Active, Unrestricted DEA license.
- BLS certification preferred.
Knowledge, Skills, & Abilities:
- Knowledge of the principles and practices of family medicine, general internal medicine, or pediatrics.
- Knowledge of the local, state, and federal regulations pertaining to medicine and to community health centers.
- Knowledge of the structure and function of community health centers.
- Excellent analytical and critical thinking skills.
- Excellent writing skills.
- Ability to learn Electronic Health Records system.
- Bilingual in one of the Santa Clara County threshold languages (Mandarin, Cantonese, Spanish, Tagalog, Farsi, or Vietnamese) preferred.
- Ability to establish and maintain effective work relationships as part of a multi-disciplinary team.

Working conditions
Work is often conducted in an office environment inside a standard clinic setting. Basic safety precautions and use of protective clothing or gear is required. Occasional evening work.
