We are seeking a compassionate and experienced Medical Social Worker to provide specialized case management, advocacy, and support services for individuals with special healthcare needs. This role involves assessing patient and family needs, coordinating medical and social services, and collaborating with healthcare providers to develop comprehensive care plans. The ideal candidate will have expertise in working with patients with disabilities, chronic illnesses, or developmental conditions.
This is an immediate opening for a full-time Medical Social Worker to work at F.E. Warren AFB in Cheyenne, WY. The position is 40 hours per week schedule, Monday through Friday, day shift only, approximately 7:30a - 4:30p.
Responsibilities:
- Conduct psychosocial assessments to determine the needs of patients with special healthcare conditions.
Develop and implement individualized care plans that integrate medical, social, and community resources.
Act as a liaison between patients, families, healthcare providers, and community organizations to ensure seamless coordination of care. - Advocate for patients and families to ensure they receive appropriate medical and social services.
Provide emotional support, crisis intervention, and counseling to patients and families.
Educate patients and caregivers on available resources, benefits, and coping strategies.
Assist families with navigating healthcare systems, insurance, and financial assistance programs.
Facilitate support groups and educational workshops for individuals with special needs and their caregivers.
Maintain accurate documentation of patient interactions, care plans, and referrals in compliance with organizational and regulatory requirements.
Stay updated on policies, laws, and best practices related to special needs populations and healthcare social work.
Qualifications:
- Degree: Master’s degree in social work (MSW).
Education: Graduate from a School of Social Work fully accredited by the Council on Social Work Education (CSWE).
Experience: Minimum of 2 years experience working with case management, care coordination, and/or social work services - Licensure: Current, full, active, and unrestricted license to practice as a Licensed Clinical Social Worker highly PREFERRED
Skills:
Strong understanding of healthcare systems, disability rights, and social service resources.
Excellent advocacy, crisis intervention, and counseling skills.
Ability to work collaboratively with interdisciplinary teams.
Strong communication, documentation, and problem-solving skills.
Knowledge of Medicaid, Medicare, and other healthcare coverage options for special needs populations.
