Responsibilities:
- Provide routine individualized care to the elderly, convalescents, or persons with disabilities
- Monitor and report changes in patient health status.
- Provide care at the direction of the registered nurse.
- Assist with exceptional care in the activities of daily living and personal care including:
- Activities of Daily Living could include: Bathing, Toileting, Hygiene care, Dressing, Feeding, Exercises, Medication reminders, etc. These responsibilities will vary based on the client's Plan of Care.
- Experience in hospice, home health, or other related fields
- Positive and patient demeanor
- Ability to build rapport with patients
- Ability to handle physical workload
- Excellent written and verbal communication skills
- Must be able to understand, read and write English
