We are seeking Full-time Primary Care Physicians for our Outpatient clinic in Waukegan, IL. The schedule is Monday-Friday 8a-5p; no nights and no weekends.
What We Offer:
- Competitive Salary (Ranges from $215,000 -$275,000 based on experience and location) with quarterly bonus based on quality metrics
- 6 weeks of PTO, inclusive of vacation, sick time, major holidays, and continuing medical education (CME)

Required Qualifications:
- National certification in at least one of the following specialties: Internal Medicine or Family Medicine
- Active, non-probationary state physician License
- 1 year direct primary care experience as a Physician
- Active DEA license
Role Summary:
- Provision of exceptional primary care
- Conduct office visits for routine and acute issues, and home visits as needed
- Administer Annual wellness visits and health risk assessments
- Care coordination with other providers, specialists, testing facilities, and agencies
- Population health leadership, in coordination with the Care Team
- Assisting the care team with phone triage and outreach
- Educating patients on their health conditions, care plans, and treatments
