Nationwide Telehealth Dementia Care Nurse Practitioners

We are seeking full-time NP's who possess bilingual skills with verbal and written fluency in Spanish to join our remote healthcare team.

Job Summary

The Telehealth Nurse Practitioner in Dementia Care is a vital member of our team, dedicated to providing specialized care for patients with dementia through telehealth platforms.

Key Responsibilities:

• Develop and implement individualized care plans for patients with dementia.
• Conduct assessments, monitor progress, and adjust treatment plans as needed.
• Provide nursing, gerontological, and neurologic care to patients with dementia.
• Cultivate strong communication and documentation skills utilizing various platforms.

Requirements:

• Graduate of a Nursing, Doctor of Nursing Practice, or NP Certificate Program.
• APRN licensure in the state of practice or ability to obtain.
• Experience working as an NP in primary care, internal medicine, neurology, or gerontology, preferably in a dementia care setting.
• Strong understanding of geriatrics and care of Geriatric/Neurology patients.
• Bilingual, with verbal and written fluency in Spanish to support a diverse patient population.
