We are seeking an experienced and compassionate Geriatric Medicine Nurse Practitioner to join our team in Mishawaka, IN. This role focuses on providing comprehensive care for the frail and elderly population, with an emphasis on long-term care and chronic condition management. The ideal candidate will have a strong background in geriatric medicine, excellent clinical skills, and a dedication to improving the quality of life for older adults. As a Geriatric Medicine Nurse Practitioner, you will provide high-quality medical care to geriatric patients, including conducting assessments, diagnosing, and developing treatment plans tailored to their needs. You will manage chronic illnesses and coordinate care to optimize health outcomes for elderly patients, ensuring their physical, emotional, and social well-being. Your duties will also include routine physical exams and wellness visits for frail and elderly patients, often in long-term care or clinical settings. You will collaborate with multidisciplinary teams, including physicians, nurses, and social workers, to deliver comprehensive and coordinated care. Additionally, you will educate patients and their families about health management, preventative care, and available resources while maintaining accurate and thorough medical records that meet regulatory and organizational standards. Qualifications : ANCC or AANP Board Certified Nurse Practitioner (required). Active Indiana state license or willingness to obtain before the start date. Minimum of 5–7 years of Nurse Practitioner experience, preferably in long-term care (does not include RN or training experience). At least 1 year of experience working with the frail and / or elderly population. BLS certification (or willingness to obtain). Controlled Substance Registration (CSR) or Controlled Dangerous Substances (CDS) certification (or willingness to obtain). DEA registration (or willingness to obtain).
