We are seeking highly skilled and motivated Full-time Nurse Practitioners to join our primary care team at our Outpatient clinic in New Haven, Connecticut.

Required Qualifications
- National Certification: Active certification in one or more specialties, including Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
- Experience: At least 1 year of direct primary care experience as a Nurse Practitioner.
- DEA License: An active DEA license.
Job Summary
The successful candidate will provide exceptional primary care services, conducting office visits, administering wellness visits, and coordinating care with other providers and specialists. They will also participate in population health leadership and assist the care team with phone triage and outreach.
