Physician (MD/DO) - Family Practice in Bronx, NY

We are seeking Full-time Primary Care Physicians for our Outpatient clinic in Bronx, NY. The schedule is Monday-Friday 8a-5p; no nights and no weekends.

• Competitive Salary (Ranges from $215,000 -$275,000 based on experience and location) with quarterly bonus based on quality metrics

Required Qualifications:

• National certification in at least one of the following specialties: Internal Medicine or Family Medicine
• Active, non-probationary state physician License
• 1 year direct primary care experience as a Physician
• Active DEA license
  
  

Role Summary:

• Provision of exceptional primary care
• Conduct office visits for routine and acute issues, and home visits as needed
• Administer Annual wellness visits and health risk assessments
• Care coordination with other providers, specialists, testing facilities, and agencies
• Population health leadership, in coordination with the Care Team
• Assisting the care team with phone triage and outreach
• Educating patients on their health conditions, care plans, and treatments
