Position Summary :

The Director of Social Services is responsible for overseeing the social services department at Abbotsford Healthcare Center. This person is respoisbile for ensuring the provision of comprehensive social work services to residents, patients, or clients. This role involves leadership, strategic planning, program development, and ensuring compliance with relevant regulations. The Director collaborates with other departments, and advocates for the needs and rights of individuals under care.

Key Responsibilities :

Leadership and Management :

Ensure that social work services are effectively integrated into the overall care plan for residents or patients.

  • Develop and implement departmental policies, procedures, and best practices to improve service delivery.

Program Development and Evaluation :

Design, implement, and assess social work programs that address the psychosocial, emotional, and social needs of residents or patients.

  • Monitor the effectiveness of services, ensuring they meet established goals and improve client outcomes.
  • Research best practices and incorporate new techniques or models of care to enhance service delivery.
  • Resident and Family Advocacy :

    Ensure residents' rights are respected and advocate for their interests in care planning and decision-making.

  • Support families in coping with complex care decisions, transitions, or crises.
  • Address and resolve any grievances or complaints from residents, families, or staff regarding social services.
  • Interdisciplinary Collaboration :

    Serve as a key member of the interdisciplinary care team, collaborating with medical, nursing, and administrative staff to develop and implement care plans.

  • Coordinate with external agencies, healthcare providers, and community resources to address residents' social needs.
  • Lead or participate in case conferences, care meetings, and discharge planning.
  • Regulatory Compliance and Documentation :

    Ensure that social services staff maintain accurate and up-to-date records in accordance with legal, regulatory, and accreditation standards (e.g., HIPAA, state licensing regulations).

  • Monitor compliance with all relevant social work and healthcare regulations and standards.
  • Prepare and submit reports as required by regulatory agencies or facility leadership.
  • Crisis Intervention and Problem-Solving :

    Provide leadership and guidance during crises, such as behavioral emergencies, conflicts, or family disputes.

  • Offer expertise in managing complex emotional, social, or mental health issues, including facilitating de-escalation and conflict resolution.
  • Budget and Resource Management :

    Develop and manage the budget for the social services department.

  • Allocate resources effectively and ensure staff have the necessary tools and support to perform their duties.
  • Qualifications :
  • Education :
  • Bachelor’s Degree in social work (SW) or social welfare from an accredited academic institution preferred.
  • At least 1-year experience in health care setting
  • Licensure : Current licensure as a Social Worker (e.g., LCSW, LMSW), in accordance with state requirements.
  • Skills :
  • Strong leadership, management, and supervisory skills.

  • In-depth knowledge of social work principles, ethics, and regulations.
  • Expertise in working with diverse populations, including older adults, individuals with chronic illness, and those with disabilities.
  • Excellent communication, interpersonal, and conflict resolution skills.
  • Ability to work in a team-oriented, collaborative environment.
  • Strong organizational and time-management abilities.
  • Proficiency in using electronic health records (EHR) systems and Microsoft Office applications.
  • Working Conditions :

  • Full-time, salaried position.
  • Occasional evening or weekend work may be required.
  • Physical Requirements :

  • Ability to lift and carry up to 20 lbs.
  • Regular walking, standing, and sitting.
  • Ability to handle high-pressure situations effectively.
