Abby Kelley Foster Charter Public School seeks an experienced, collaborative, and energetic teacher to join the district for a position as the BRYT Clinical Coordinator-Long Term Substitute. Abby Kelley Foster Charter Public School (AKFCS) provides an education to 1426 students in grades kindergarten through twelve, comprised of an Elementary, Middle and High School. Position Summary: The BRYT Clinical Coordinator Long- Term Substitute is primarily responsible for providing short/medium term support to students with significant social, emotional and behavioral needs, and to these students’ families.
●Operate and continuously improve on intensive intervention dedicated to reducing the significant social, emotional and behavioral needs for students at AKF.
●Develops and ensures effective implementation of a comprehensive transition plan integrating clinical, academic and family support as well as care coordination for each student in BRYT; has the primary responsibility for implementation of the clinical, care coordination and family support components of the overall plan.
●Organizes and facilitates meetings with staff of collateral agencies prior to and during students’ participation in BRYT.
●Provides 1:1 and/or small-group clinical intervention to BRYT students on a flexible, as-needed basis, employing clinical methods that are most appropriate for meeting the goals in each student’s case management plan.
●Pro-actively communicates with educator colleagues and school administrators to keep them apprised of student progress and gather feedback and information to support student learning and well-being, including making home visits to families where appropriate; develops and provides family support and engagement activities.
●Maintains a program environment that is conducive to learning and appropriate to the maturity and interest of students.
●Ensures that all required data is gathered and entered data management systems according to program procedures.
●Develops reasonable rules of classroom behavior and procedure, and maintains order in the classroom, including addressing inappropriate student behaviors as needed.
●Any other duties as assigned in relation to the function of the BRYT program.

●Strong clinical skills demonstrated in an outpatient or school setting.
●Excellent case management skills.
● Ability to coordinate services across systems.
●Excellent communication skills (verbal and written).
●Proficiency with documentation in an electronic health record.
●Proven ability to collaborate with providers, families, and social service agencies.
● Desire to develop and maintain collaborative relationships.
●Excellent organizational skills.
