We are looking for…

An experienced bilingual case manager with a heart for children and families with Special Healthcare Needs who live in San Antonio, TX. Previous experience serving children with special healthcare needs is strongly preferred, but other related experiences will be considered.

  • Awareness and respect for the diverse cultural backgrounds of the families served, ensuring inclusive and respectful support.
  • Knowledge of community resources and ability to connect families with necessary services and support.
  • Ability to identify issues and develop effective, individualized solutions for families' needs.
  • Proficiency in managing multiple cases, maintaining accurate records, and organizing documentation.
  • Ability to prioritize tasks and manage time efficiently to serve multiple families effectively.
  • Strong ability to advocate for the needs and rights of clients within various systems and institutions.
  • Emotional strength and self-care practices to handle the demands of working with families in challenging situations and maintain personal well-being.
  • Ability to work effectively in a team environment and contribute to a positive organizational culture.
  • Bachelor's degree in a relevant field
  • 2 years of related experience working in case management or community outreach services within community programs serving families with special healthcare needs.
  • Bilingual English/Spanish
  • Valid Texas Driver License

Job Duties

Your role will include establishing comprehensive service plans that provide referrals and direct assistance customized to meet the specific needs of each child and family. You will review and update family service plans quarterly and annually, ensuring progress notes are current and modifications are made as needed. You will also stay informed about local, state, and national resources, utilizing these resources to benefit your clients.

Assist families in navigating and completing applications for insurance and funding programs, including but not limited to CSHCN, SSI, and Medicaid waivers. Apply evidence-based practices in service delivery, such as Person-Centered Planning, Trauma-Informed Care, and the CSHCN Practice Model. Manage client contacts, case files, and the ClientTrack database, ensuring timely updates, thorough documentation, and accurate information.

Conduct client interviews in private settings—whether at home, in the office, or at alternate sites—to ensure privacy and confidentiality. Protect sensitive information obtained through the course of your work. Utilize pre-surveys to guide the development of service plans for active cases and post-surveys for cases pending deactivation.

Maintain an average client caseload as designated by program requirements. Participate in ongoing training, community networking, and agency courses related to resources available to families in need. Follow the approval process for any expenses incurred and contribute to the planning, coordinating, and attending agency-sponsored events. Provide direct and indirect support to fundraising efforts and perform other duties, tasks, and special projects as assigned.
