Community-Based Adult Services / Adult Day Health Care facility looking for an MSW or Social Worker Assistant. Experience in ADHC is a PLUS.

Masters Social Worker :

  • Masters in Social Work; OR
  • Masters in psychology, gerontology or counseling with ONE year of experience providing social services; OR,
  • Bachelor's degree in social work with TWO years of experience providing social services.

Social Worker Assistant :

Applicant must have a Bachelor's Degree in a Social Work, Psychology, Sociology or Social Welfare from an accredited college or university.

MSW / SW Assistant Job Duties :

  • Provide assessment and reassessment
  • Development and implementation of Individual Plan of Care
  • Individual and group counseling to elderly participants as per each individual plan of care
  • Planning and coordination of discharge plan
  • Documentation of provided services
  • Provide various care management services to participants in order to obtain needed services such as IHSS and Access
  • Assist with Primary Care Physician appointments
  • Provide case management to participants in need of housing referrals and access to social welfare programs
  • Complete all relevant paperwork and maintain participants' files
  • MSW starting salary at $22 per hour / DOE.
  • SW assistant starting salary at $18 per hour / DOE.
