We are seeking an experienced and detail-oriented Service Coordinator to oversee and manage homecare services for individuals in the Nursing Home Transition & Diversion (NHTD) program. The ideal candidate will have a strong understanding of NHTD procedures, policies, and compliance requirements in homecare settings.Responsibilities : Coordinate and manage homecare services for NHTD clients, ensuring quality care and compliance.Serve as the primary point of contact for clients, families, and caregivers.Develop, implement, and monitor individualized service plans.Ensure compliance with state regulations and program policies.Communicate effectively with healthcare professionals, service providers, and agencies.Maintain accurate documentation and reports on client care and service coordination.Identify and address any service gaps to enhance client support.Requirements : Prior experience in NHTD, TBI waiver programs, or homecare service coordination.Strong knowledge of NYS DOH regulations and Medicaid-funded programs.Excellent organizational and communication skills.
