What are the responsibilities and job description for the HCS Group Home Case Manager position at Abiding Choice?
Group Home Managers direct operations in residential homes for individuals with Intellectual and Developmental Disabilities. Common tasks include overseeing daily operations, developing budgets, hiring and training staff, scheduling employees, and implementing programs for residents. Essential assets for this position include managerial skills, leadership, communication and interpersonal abilities, budgeting skills, multi-tasking, and teamwork. This position will require some travel from Belton, Texas to Rockdale, Texas.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Participate as Service Planning team member and CAAC/Consumer Advocate Advisory Committee as assigned.
- This position will be a co-manager of homes with our already on staff Group Home Manager.
- Ensure ongoing active treatment-observe training at least once each month at each group home as assigned.
- Monitors and maintain compliance with HCS standards/HCS Principles,
- Participates in on-call duties
- Participate in annual state//federal surveys, follow up reviews and all investigations regarding abuse/neglect/exploitation, and maintains investigations records.
- Ensure adequate staffing coverage as it pertains to your job position.
- Provide Case Management assigned duties for all assigned clients and may be required to assist other case managers if needs arise.
- Ensuring compliance in all areas of the home (shopping, supplies, appointments, maintenance needs)
- Identify Abuse/Neglect/Exploitation and follow proper procedures for reporting.
- Maintain ongoing communication with local, state and federal entities, including but not limited to the Local Authority, guardians, LARs, family members, Host Home Providers, and a wide variety of professionals in the community.
- Cooperate and assist, as needed, with legal status assessments including guardianship and advocacy needs.
- Monitor trust-fund requests.
- Update and monitor information for the people on assigned caseload
- Review and follow-up on incident reports.
- Make chart audit correction as needed
- Respond to emergency situations involving people on assigned caseload.
- Other tasks may be assigned as needs arise.

- Must be self-directed with strong organizational and time management skills.
- Must be proficient in Microsoft Computer Applications.

Pay: $18.00 - $22.00 per hour
