Essential Functions and Key Responsibilities:

  • Manage a Diverse Caseload: Oversee a dynamic and varied caseload, balancing complexity and individual needs to deliver tailored support.
  • Engage with Clients: Conduct impactful interviews with individuals, families, and stakeholders to gather essential data and complete necessary documentation.
  • Document and Review: Prepare detailed case notes, quarterly and annual reviews, and service monitoring reports, ensuring compliance with Department of Mental Health (DMH) guidelines and Medicaid regulations.
  • Timely Documentation: Efficiently complete all required documentation, including service plans and waiver paperwork, adhering to DMH, Medicaid, and Abilities First policies.
  • Utilize Technology: Accurately enter case notes into electronic record system, maintaining productivity standards of 120 TCM hours per month and 1,440 hours annually.
  • Maintain Records: Keep individual state records up to date in accordance with DMH and Medicaid guidelines, ensuring accuracy and accessibility.
  • Facilitate Team Collaboration: Lead the interdisciplinary team in developing and reviewing Individual Service Plans (ISPs), ensuring all perspectives are considered.
  • Monitor Service Delivery: Oversee services to ensure they meet authorization terms and align with state directives.
  • Set Goals and Outcomes: Collaborate with individuals to identify evolving needs and develop personalized goals, while connecting them to community resources and natural supports.
  • Regular Contact: Maintain consistent communication through monthly and quarterly face-to-face visits and service monitoring in line with DMH guidelines.
  • Verify Service Accuracy: Meet with individuals to verify documentation and ensure service accuracy with contracted providers.
  • Conduct Assessments: Perform individual assessments utilizing specialized knowledge from fields such as social work, psychology, or counseling as needed.
  • Manage Budgets: Prepare and maintain accurate expenditure records for individuals, ensuring alignment with their Individual Service Plan budgets.
  • Handle Emergencies: Be ready to respond to emergency situations, following Abilities First policy and state regulations, including mandated reporting duties.
