What are the responsibilities and job description for the Clinical Team Support position at Able Palms Home & Health Care Services Inc?
Processes all scheduling related activities from intake to discharge.
1. Demonstrate professional conduct and ethics according to agency policy and
procedure.
2. Follow and adhere to confidentiality policy in regards to all clients and agency
operations.
3. Review eligibility verification to ensure Medicare is active and client coverage meets
company policy.
4. Receive initial patient referral information and process intake task as required if
needed to assist intake coordinator.
5. Process all schedule related tasks as workflow requires.
6. Review, process and document all missed visit tasks as workflow requires.
7. Review coordination notes as assigned.
8. Perform clerical tasks as needed.
9. Maintain excellent level of customer service when interacting with internal and
external customers.
10. Maintain ongoing effective communication with other members of the care team.
11. Perform other job duties and responsibilities as assigned by the Administrator
including participating in performance improvement activities; i.e.
performance improvement groups.
1. Graduate of vocational/technical school with current license in respective state
2. Minimum one (1) year experience and/or training in home health care preferred.
3. Background check acceptable to the Agency.
4. Able to work autonomously with minimum supervision.
5. Demonstrated organizational skills and detail-oriented.
6. Demonstrated computer skills.
7. Demonstrated time management skills.
8. Demonstrated interpersonal skills.
9. Demonstrated verbal and written communication skills.
10. Able to communicate in English.
11. Able to work as a team member.
