Able2 is currently searching for candidates for our Program Coordinator position. The Program Coordinator is responsible for working with the planning team to develop Staff Action Plans, Individual Plans of Protective Oversight, Behavior Support Plans, Care Plans, and other necessary plans. The Program Coordinator monitors the implementation and documentation of plans to ensure accurate and timely submission of billing and works with others to ensure integration of plans across programs as appropriate.

Our Program Coordinator pay range is $20.30-22.40. Pay is based on relative experience and education. The schedule for this vacancy is Monday-Friday, day shift.
Responsibilities:
- Assists Director/Manager in completion of intake/admission processes ensuring necessary enrollment and service authorization documentation is obtained and that program registration paperwork is submitted as required.
- Builds rapport with caregivers/families/guardians/advocates to elicit and share information relevant to the individual’s welfare and programming. Advocates for individual rights and interests and assists individual and family/guardian/advocate in making informed decisions and advocating for themselves.
- Maintains regular contact with caregivers and other professionals providing services to the individual, and updates records as appropriate. Acts as liaison between residential/day program and other service providers. Maintains at least monthly contact with the Care Coordinator and/or other Program or Hab Coordinators who support the person to ensure coordinated approach to supports and planning.
- Coordinates, facilitates and attends Life Plan/IPP meetings as scheduled, supplying recommendations based upon consideration of individual’s progress and identified value outcomes, assessments, etc.
- In conjunction with Care Coordinator and Program Management, monitors individuals’ finances, benefits, personal allowance use and services to ensure that established benefits and services are maintained (clinical services, Medicaid, food stamps, etc.) Assists with recertification of benefits when appropriate.
Qualifications:
- Bachelor’s degree in education, social work, psychology, rehabilitation or related human service field from accredited program, or holds certification from NYS Dept. of Education.
- Has specialized training or one years’ experience in treating or working with individuals with developmental disabilities.
- Experience with habilitation services in a residential or day program setting preferred.
- Possess a valid driver’s license and/or have access to reliable transportation.
