Directly responsible to the Manager for providing direct support activities and supervision of the persons served to help them achieve their personal goals.
Shift: Sunday-Tuesday, 7am-7pm, Wednesday 11a-3pmRequirements:
High school diploma or equivalent required.
At least 2 years of experience as a Direct Support Professional or related job working with persons with developmental disabilities.
6 months or more previous leadership experience preferred
Must be at least 18 years of age.
Valid driver's license and clear driving record
Cleared and approved criminal record check
First Aid/CPR Certified
Ability to assist with a two-person lift of an adult
Ability to stand for eight (8) hours at a time.

Responsibilities:
Ensures the ISP maximizes the quality of life, supporting engaging activities to enhance the quality of daily living, individuals’ personal goals, and spiritual supports.
Writes and enters service delivery documentation, support others in entry of information into system.
Ensures general event reporting (GER) is maintained and is aligned with medical orders, state compliance requirements, abuse reporting, court orders and AbleLight quality and internal procedures and guidelines
Delivers holistic person-centered care daily care, with dignity and respect and aligned with the Individual Support Plan (ISP)
Provides personal care such as bathing, dressing, bathroom assistance or housekeeping.
Prepares and serves meals in accordance with individual diet plans.
Administers medication as documented under the direction (supervision) of a nurse.
Schedules and take individuals to medical appointments, outings, or errands.
Attending staff meetings and participating in continuing education.
Audits of health appointments, individual support plans, monitoring medical supplies and prescriptions, and monthly facility audits.
Ordering food and supplies and allocation of receipts to house and program financials.
Functions as a front-line resource for DSP colleagues, persons supported, or parent & guardians in the home.
