About the Role:
- As an Advanced Care Practitioner with Abode Care Partners, you will have the opportunity to work with a talented team of healthcare professionals to deliver exceptional care to patients in need. Your role will involve providing evidence-based medical care, examining and treating chronic and urgent episodic illnesses, and collaborating with caregivers and other healthcare professionals to ensure seamless coordination of care.
- Key requirements for this role include a Master's degree in Nursing (MSN), at least five years of nursing experience, proficiency in medical instruments and equipment, and experience with computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications.
Our competitive compensation package includes a salary range between $80,000 and $110,000, with total earnings potential reaching $120,000 through our monthly incentive plan based on performance.
