At Abode Care Partners, we're committed to delivering exceptional medical care to individuals from post hospitalization to home. As an Advanced Practitioner, you'll play a vital role in our team by providing evidence-based medical care to seniors and patients with complex needs.
Key responsibilities include examining and treating chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses. You'll also be responsible for ordering and interpreting laboratory and diagnostic testing as needed, collaborating with caregivers and other healthcare professionals in providing interdisciplinary care, and maintaining a comprehensive plan of care.
To be successful in this role, you'll need to possess strong interpersonal communication skills β verbal and written, excellent time management skills with accountability for practitioner's daily workflow, and proficiency in medical instruments and equipment. A Master's degree in Nursing (MSN) is required, along with active state Nurse Practitioners license, current Nurse Practitioner National Board Certification, and active state DEA licensure.
