We are seeking a dedicated APRN Medical Care Provider to join our team at Abode Care Partners. As a key member of our healthcare team, you will provide high-quality medical care to seniors and patients with complex needs in various settings.
This role requires an active APRN license, a Master's degree in Nursing, and strong interpersonal and time management skills.
Key Responsibilities:
- Develop and implement individualized plans of care that meet the unique needs of each patient
- Collaborate with caregivers and other healthcare professionals to ensure seamless coordination of care
- Provide education and support to patients and their families to promote healthy behaviors and self-management
- Maintain accurate and detailed patient records to ensure continuity of care
Requirements:
- Active state APRN license
- Current APRN National Board Certification
- Active state DEA licensure
- Master's degree in Nursing (MSN)
- Five or more years of nursing experience; at least two years of APRN experience preferred
- Strong interpersonal communication skills – verbal and written
- Excellent time management skills with accountability for practitioner's daily workflow
