Job Responsibilities:
- Develop and implement individualized plans of care for patients with chronic and acute conditions
- Conduct thorough assessments and evaluations to determine the best course of treatment
- Order and interpret laboratory and diagnostic testing as needed
- Collaborate with healthcare teams to ensure seamless transitions of care
- Provide education to patients and families on disease management and self-care techniques
- Stay up-to-date with the latest medical research and developments in the field of nursing
Qualifications:
- Current state Nurse Practitioner license
- National Board Certification as a Nurse Practitioner
- Active state DEA licensure
- Master's degree in Nursing (MSN)
- Minimum five years of nursing experience; at least two years of nurse practitioner experience preferred
- Strong communication and interpersonal skills
- Proficiency in electronic medical records and Microsoft Office
- Knowledge of HIPAA and OSHA regulations
- Ability to lift up to 25 pounds and stand/walk for extended periods
