This is a full-time opportunity for a Nurse Practitioner to join our team of dedicated healthcare professionals. The ideal candidate will have strong interpersonal communication skills and experience working in home healthcare settings.
Responsibilities:
- Evaluate and treat patients with chronic and acute illnesses, injuries, and other conditions.
- Collaborate with caregivers and other healthcare professionals to develop comprehensive plans of care.
- Provide referrals to specialists when necessary and ensure continuity of care.
- Maintain accurate and up-to-date patient medical records.
Requirements:
- Active state Nurse Practitioners license.
- Current Nurse Practitioner National Board Certification.
- Master's degree in Nursing (MSN).
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
- Strong knowledge base of HIPAA and OSHA regulations.
- Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications.
