Job Description
We are seeking an experienced Senior Nurse Advisor to join our team at Abode Care Partners. As a member of our team, you will play a vital role in providing high-quality medical care to patients with chronic and complex needs.
Responsibilities
- Develop and implement individualized care plans for patients with chronic and complex needs
- Conduct routine health assessments and order laboratory and diagnostic testing as needed
- Collaborate with caregivers and other healthcare professionals to maintain comprehensive care plans
- Provide referrals to other healthcare practitioners when necessary
Requirements
Master's degree in Nursing (MSN)
Five or more years of nursing experience; Proficiency in medical instruments and equipment
Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications
