**Responsibilities and Qualifications:**
- Provide evidence-based medical care to patients with chronic and urgent episodic illnesses
- Collaborate with caregivers and other healthcare professionals to develop comprehensive plans of care
- Manage patient records and ensure ongoing patient care through assessments and referrals
- Active state Nurse Practitioners license and current Nurse Practitioner National Board Certification
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
