About the Role
This full-time position offers a chance to work with a diverse patient population, providing personalized care in home and community settings. You will be responsible for conducting medical assessments, developing treatment plans, and collaborating with healthcare professionals to ensure seamless care coordination.Key Accountabilities
- Patient Assessment and Care Planning: Conduct thorough medical assessments to identify patients' needs and develop effective treatment plans.
- Medication Management: Order and administer medications as prescribed by physicians, ensuring safe and effective use.
- Care Coordination: Collaborate with caregivers, physicians, and other healthcare professionals to ensure comprehensive care and timely communication.
- Documentation and Record-Keeping: Maintain accurate and up-to-date patient medical records, adhering to regulatory requirements.
Qualifications and Requirements
- Licensure and Certification: Active state Nurse Practitioner license and current National Board Certification.
- Education and Experience: Master's degree in Nursing (MSN) and at least five years of nursing experience, with two years of nurse practitioner experience preferred.
- Technical Skills: Proficiency in medical instruments, electronic medical record (EMR) systems, and Microsoft Office applications.
