Job Description
We are seeking an experienced Adult Gerontology Nurse Practitioner to join our team. As a key member of our healthcare team, you will provide compassionate, evidence-based medical care to seniors and patients with complex needs in their home or community settings.
Responsibilities:
- Deliver high-quality medical care to patients in various settings
- Collaborate with healthcare professionals to ensure comprehensive patient care
- Maintain accurate and up-to-date patient medical records
- Provide referrals to other healthcare practitioners when necessary
Requirements:
- Active state Nurse Practitioners license
- Current Nurse Practitioner National Board Certification
- Active state DEA licensure
- Master's degree in Nursing (MSN)
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
