Job Description:
We are seeking a compassionate Nurse Practitioner to join our team. As a key member of our care team, you will be responsible for providing evidence-based medical care to our patients. You will work closely with our interdisciplinary team to develop comprehensive care plans and ensure that our patients receive the best possible care.
Key Responsibilities:
- Perform routine medical checks and treat chronic and urgent episodic illnesses
- Collaborate with caregivers and other healthcare professionals to develop comprehensive care plans
- Provide referrals to other healthcare practitioners when necessary
- Maintain accurate patient medical records
Requirements:
- Active state Nurse Practitioners license
- Current Nurse Practitioner National Board Certification
- Active state DEA licensure
- Master's degree in Nursing (MSN)
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
