Evidence-Based Medical Care Professional Needed
We are excited to announce a new opportunity for a skilled Nurse Practitioner to join our team at Abode Care Partners. As a key member of our interdisciplinary care team, you will provide high-quality medical care to seniors and individuals with complex needs.
Your responsibilities will include examining and treating chronic and urgent episodic illnesses, minor injuries, and other health concerns. You will also collaborate with caregivers and other healthcare professionals to maintain comprehensive patient plans and ensure seamless care transitions.
A successful candidate will possess strong knowledge of evidence-based medicine, effective communication skills, and ability to work collaboratively as part of a healthcare team. A Master's degree in Nursing (MSN) and current Nurse Practitioner National Board Certification are required.
