About the Role
The Advanced Practice Nurse (APN) at Abode Care Partners plays a pivotal role in delivering high-quality medical care to seniors and patients with complex needs in their homes and assisted living facilities. As a seasoned healthcare professional, you will be responsible for providing personalized medical services that cater to the unique requirements of each patient.
Key responsibilities include performing comprehensive medical assessments, treating chronic and urgent illnesses, and collaborating with other healthcare professionals to ensure seamless care coordination. You will work closely with our interdisciplinary team to develop and implement customized care plans that prioritize patient well-being and safety.
To excel in this role, you will possess a Master's degree in Nursing, active APN licensure, and strong communication skills. Experience working with seniors and patients with complex needs is highly desirable.
Responsibilities:
- Provide evidence-based medical care by planning, directing, and evaluating patient medical needs
- Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses
- Order and interpret laboratory and diagnostic testing as needed
- Collaborate with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care
- Provide referrals to other healthcare practitioners when necessary
- Ensure patient medical records are kept up to date
Qualifications:
- Active state APN licensure
- Current APN National Board Certification
- Active state DEA licensure
- Master's degree in Nursing (MSN)
- Five or more years of nursing experience; at least two years of APN experience preferred
- Strong interpersonal communication skills – verbal and written
- Excellent time management skills with accountability for practitioner's daily workflow
- Proficiency in medical instruments and equipment
- Strong knowledge base of HIPAA and OSHA regulations
- Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications
- Licensed driver with valid automobile insurance
- Ability to engage in moderate physical activity, such as handling objects up to 25 lbs., standing or walking for more than four hours per day, etc.
