Job Summary: We are seeking a compassionate Nurse Practitioner to join our team at Abode Care Partners. As a key member of our healthcare team, you will provide evidence-based medical care to seniors and patients with complex needs.
The ideal candidate will have a Master's degree in Nursing (MSN) and be licensed as a Nurse Practitioner (NP). You will work collaboratively with caregivers and other healthcare professionals to develop and implement comprehensive plans of care for our patients.
Responsibilities:
- Provide primary and specialty care to patients with chronic and urgent episodic illnesses, minor injuries, accidents, and other health issues
- Order and interpret laboratory and diagnostic testing as needed to ensure accurate diagnoses and effective treatment plans
- Collaborate with caregivers and other healthcare professionals to maintain a comprehensive plan of care
- Provide referrals to other healthcare practitioners when necessary
