About the Role
We are seeking a skilled and compassionate Senior Healthcare Professional to join our team at Abode Care Partners. As a key member of our healthcare team, you will provide evidence-based medical care to seniors and patients with complex needs in various settings, including their homes. Your expertise will be essential in providing comprehensive patient care, collaborating with healthcare professionals, and ensuring seamless transitions across care settings.
Your Responsibilities Will Include:
- Conducting thorough examinations and treating chronic and urgent illnesses, minor injuries, accidents, and other health issues.
- Ordering and interpreting laboratory and diagnostic testing as needed, ensuring accurate diagnoses and effective treatment plans.
- Collaborating with caregivers and other healthcare professionals to provide interdisciplinary care and maintain a comprehensive plan of care for each patient.
- Providing referrals to other healthcare practitioners when necessary, promoting a seamless continuum of care.
- Ensuring patient medical records are up-to-date and accurate, maintaining confidentiality and adhering to HIPAA regulations.
You Will Be Expected To Have:
- An active state Nurse Practitioners license and current National Board Certification.
- Active state DEA licensure and a Master's degree in Nursing (MSN).
- Five or more years of nursing experience, with at least two years of nurse practitioner experience preferred.
- Strong interpersonal communication skills – verbal and written – with the ability to engage with diverse populations.
- Excellent time management skills with accountability for practitioner's daily workflow.
- Proficiency in medical instruments and equipment, as well as knowledge of HIPAA and OSHA regulations.
- Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications.
- A valid driver's license and automobile insurance, with the ability to engage in moderate physical activity.
