Overview
Abode Care Partners is a leading provider of integrated medical services, caring for individuals from post-hospitalization to home in various settings. Our team brings quality medical care to seniors, people with complex conditions, special needs, and individuals with intellectual and/or developmental disabilities.
Responsibilities
- Provide high-quality complex and geriatric care to residents and patients across settings in partnership with non-physician practitioners and other interprofessional team members.
- Keep high engagement with facility leadership teams, nurse practitioners/physician assistants, patients, and families.
- Provides collaborating physician duties for on average of 5 but up to 15 nurse practitioners and physician assistants (based on state regulatory parameters) to support them in their clinical roles.
- Audit up to 10% of practitioner medical orders and/or documentation for each provider monthly.
- Assume medical directorships for skilled and/or other senior living facilities as needed.
- Help identify new growth opportunities with skilled facilities and/or other senior living facilities in the market/region.
- Participate in company frontline/backup call processes for timely response to acute changes of condition to prevent avoidable hospital transfers, or to meet other clinical needs.
