Part-time Nurse Practitioner

Job Title: Nurse Practitioner - APRN

About the Role:

We are seeking a highly skilled Nurse Practitioner to join our team at Abode Care Partners. As a key member of our healthcare team, you will provide evidence-based medical care to seniors and patients with complex needs.

Your Responsibilities:

• Perform examinations and create comprehensive care plans in collaboration with healthcare professionals.
• Provide personalized care to seniors and patients with complex needs where they reside.
• Maintain accurate medical records and ensure patient confidentiality.
• Order and interpret laboratory and diagnostic testing as needed.
• Collaborate with caregivers and other healthcare professionals to provide interdisciplinary care.

Requirements:

• Master's degree in Nursing (MSN)
• Five or more years of nursing experience
• Proficiency in medical instruments and equipment
• Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications
