Job Overview
This role involves providing evidence-based medical care to seniors and patients with complex needs in various settings, such as skilled nursing facilities, assisted living, independent living, group homes, and private homes.
Responsibilities:
- Perform comprehensive assessments of patient medical needs and develop personalized treatment plans
- Examine and treat chronic and urgent health issues, minor injuries, accidents, and other illnesses or injuries
- Order and interpret laboratory and diagnostic testing as needed
- Collaborate with caregivers and other healthcare professionals to provide interdisciplinary care and maintain comprehensive patient records
- Provide referrals to other healthcare practitioners when necessary
Qualifications:
- Active state Nurse Practitioners license
- Current Nurse Practitioner National Board Certification
- Active state DEA licensure
- Master's degree in Nursing (MSN)
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
- Strong interpersonal communication skills β verbal and written
- Excellent time management skills with accountability for practitioner's daily workflow
- Proficiency in medical instruments and equipment
- Knowledge base of HIPAA and OSHA regulations
- Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications
